Very RARE from my research. The only photo I have found on the web is from the museum of Vancouver.
Wired for USA and Canada. Manufactured by W. Freeman and Sons (Established 1930), Vancouver, Canada.

Double-headed table lamp; two tulip-like black painted flat metal leaves, one tall one short, are attached at centre of round brass base; two thin brass stems rise up from centre of leaves, cross each other and reach out above the 'leaves', one taller than the other; light green coloured fiberglass shades are attached on ball swivel at the end of each stem; electrical cord exits base at back; original gold, red, black and cream sticker on base, 'Manufactured by W. Freeman and Son, established 1930, Canada'.
Material
Dimensions: 24 7/8" high x 24" wide; base 7 5/8" diameter; shades 6 1/8" long x 6" diameter; 13" from leaf tip to leaf tip.
Condition: Very good vintage condition. Very clean and shiny as seen in the photographs. Some signs of wear consistent with its decades of typical use. Such as light scratches, scuffs and some oxidization. Original wiring photographed, however the cord will be replaced. Please refer to all photographs closely (ZOOM IN) for more condition information.
